MV3001 Form Wisconsin: What It Is and How to Complete It for Reinstatement

What Form MV3001 Is

Form MV3001 is the standard Wisconsin driver license application. You need it any time you're applying for a new license, renewing, or — critically — reinstating a suspended or revoked license. It's the form the DMV service center requires when you walk in to restore your driving privileges.

Download MV3001 from WisDOT's forms page at wisconsindot.gov under the "Forms & Documents" section. Print it and fill it out before your DMV visit — completing it at the counter takes time and can hold up your appointment.

What You Need to Bring

The MV3001 is a starting document, but the DMV also requires physical proofs of identity and legal presence at the time of your visit:

  • Proof of identity: An unexpired U.S. passport, birth certificate, or certificate of naturalization. A previous Wisconsin license alone may not be sufficient for reinstatement.
  • Proof of Wisconsin residency: Two documents showing your current address — utility bills, bank statements, or lease agreements work.
  • Social Security Number: Required for the application. If you don't have your card, a W-2 or SSA-1099 showing the full number is accepted.

If you're reinstating after an OWI, you'll also need to ensure your SR-22 has been filed electronically by your insurer and that any court-ordered IID installation has been reported to the DMV's database before you arrive.

Form MV3027: The Occupational License Schedule

If you're applying for an occupational license alongside your MV3001, you'll also need Form MV3027. This is the schedule form where you specify exactly when and why you need to drive — work hours, school schedule, medical appointments, essential household tasks.

MV3027 requires precision. You must list specific hours of operation within the 12-hours-per-day, 60-hours-per-week limit. A vague schedule like "mornings and evenings" won't be accepted. State your actual shift times, commute durations, and the specific purposes for each driving window.

Once the DMV approves your MV3027 schedule, changing it costs $50 per modification. The only free change is a one-time modification needed to comply with your Driver Safety Plan.

Common Mistakes

Going to the DMV before clearing all holds. The MV3001 application can't be processed if you still have unresolved court debt, child support holds, or out-of-state flags. Check the WisDOT online portal first.

Arriving too late. Walk-in reinstatement processing takes time. Arrive at least two hours before the service center closes to ensure same-day processing.

Missing documents. Without the required identity and residency proofs, the DMV will turn you away regardless of whether your holds are cleared. Double-check the document requirements before your trip.
