For unpaid court fines or citations: A court-issued "Verification of Enrollment" or "Verification of Satisfaction" form from each municipal, magistrate, or circuit court that issued a hold. This is the form that triggers a fee waiver at the DMV — without it, you'll pay the full $50 reinstatement fee for each hold.

For DUI revocations: A certified Certificate of Completion from the DMV-supervised Safety and Treatment Program (Level I Education at minimum, plus Level II or III if clinically mandated).
