Massachusetts License Reinstatement Checklist: Documents You Need

Before You Start: Pull Your Full Driving History

The biggest document mistake is starting the reinstatement process without knowing every hold on your record. Massachusetts offers three levels of driving records, and the basic one hides the holds that cause the most delays.

Full certified driving history (Merit Rating Board): This is the one to get. Submit the "Authorized Release of Personal Driving History" form to the RMV Court Records Department. It exposes everything — child support holds, drug offense suspensions, default warrants, and out-of-state blocks that don't appear on standard records. Cost: $20.

True and attested driving record: The official certified version ($20) with the Registrar's stamp. Required if you're attending a Board of Appeal hearing. Available online through myRMV (mailed to your address of record, allow 10 business days) or in person at the Chinatown/Boston or Haymarket service centers.

Unattested driving record: The basic $8 version available online through myRMV. Fine for a quick status check, but it misses hidden holds and isn't accepted for hearings.

Core Reinstatement Documents (All Suspension Types)

Regardless of why your license was suspended, you'll need:

  • [ ] Full certified driving history identifying every active hold
  • [ ] Payment for all applicable reinstatement fees (see fee schedule by suspension type)
  • [ ] Valid identification (Real ID, passport, or other accepted document)
  • [ ] Proof of Massachusetts residency if your address has changed

OUI-Related Suspensions — Additional Documents

  • [ ] Treatment program completion certificate on program letterhead (24D alcohol education for first offense, 14-day inpatient for second, 90-day inpatient for third/fourth)
  • [ ] Discharge summary with recidivism risk assessment (second offense and higher)
  • [ ] Aftercare compliance documentation (second offense and higher)
  • [ ] Probation clearance letter dated within 30 days of hearing (if on probation)
  • [ ] IID Operator's Affidavit (if ignition interlock is required)
  • [ ] Household Licensed Resident IID Affidavit — signed by every licensed driver in your household (if IID required)
  • [ ] Proof of IID installation in every vehicle you own, lease, or operate

Hardship License Applications — Additional Documents

  • [ ] Employer work letter on official letterhead, hand-signed in ink, dated within 30 days, stating specific hours and driving necessity
  • [ ] OR self-employment documentation: municipal business certificate, Schedule C, professional license, written narrative
  • [ ] OR medical documentation: physician's letter with treatment frequency and location
  • [ ] OR educational documentation: registrar enrollment verification with class schedule
  • [ ] Transit insufficiency evidence: printed bus/rail routes, MBTA schedules, Google Maps transit screenshots showing no viable public transit option
  • [ ] $50 Board of Appeal filing fee (check or money order) if appealing an RMV denial

Court Default / Warrant Holds

  • [ ] Physical green court release form OR supplemental court abstract from the issuing court
  • [ ] Proof of warrant recall fee payment ($50 per G.L. c. 276, § 30)
  • [ ] Confirmation that the court has transmitted the electronic clearance to the RMV

Out-of-State Holds

  • [ ] Certified clearance letter from the originating state showing driving privilege fully restored
  • [ ] OR official driving record from the other state showing no active suspensions
  • [ ] Documents must be dated within 30 days of your Massachusetts reinstatement request

Child Support Holds

  • [ ] DOR electronic clearance transmitted to the RMV (contact DOR Child Support Services to verify)
  • [ ] Proof of arrears payment or approved payment agreement with the DOR
  • [ ] Note: the RMV cannot accept any reinstatement payment until the DOR clearance is in their system

Retesting Requirements

If your license has been suspended or revoked for two years or longer:

  • [ ] Pass the Class D written knowledge test
  • [ ] Pass the road driving test
  • [ ] Schedule both through the myRMV appointment system

The Order Matters

Documents aren't useful if you collect them in the wrong sequence. The DOR must clear child support holds before the RMV will process anything. Courts must transmit warrant clearances before the RMV will accept fee payments for those events. Out-of-state clearance letters expire after 30 days.
